SYNCARE is the mission-oriented care coordination and telemedicine service platform for the hospitals and clinics to operate extensive services for patients from a visit to visit. With the modular care plan organiser, care managers design the healthcare plans for different patients. After subscribing to their app, patients can accomplish missions from the care plan, send their meal photo or measured data to consult with the care manager and look up the relevant healthcare articles to improve their health and stamina.
